Informed Consent & Waiver

Description of Treatment:
I understand that the muscle treatment provided may include techniques such as myofascial release, manual manipulation, stretching, or other non-invasive methods aimed at addressing muscle tension, improving mobility, or promoting relaxation. This treatment is offered as a complimentary or promotional service and is not intended as a substitute for professional medical care, diagnosis, or treatment.

Benefits and Risks:
Potential benefits may include pain relief, improved circulation, and improved range of motion and mobility. However, as with any physical intervention, there are potential risks, which may include but are not limited to: soreness, bruising, or discomfort in the treated areas. I acknowledge that I have been informed of these potential benefits and risks and have had the opportunity to ask questions.

Release of Liability:
In consideration of receiving this treatment, I hereby release, waive, and discharge Westy Sports Chiro/Dr. Jon Denning from any and all liability, claims, demands, actions, or causes of action arising out of or related to any loss, damage, injury, or harm that may occur as a result of the treatment. I agree not to hold the provider responsible for any pre-existing conditions or unforeseen reactions.
